Folate / L-Methylfolate (5-MTHF)
Required for nucleotide synthesis and cell division, critical during pregnancy and growth.
Supports neurotransmitter production and reduces depression risk, especially with MTHFR mutations.
Lowers homocysteine levels, reducing cardiovascular disease risk.
B12 and folate work together in methylation. Supplementing one without the other can mask B12 deficiency.
L-Methylfolate (5-MTHF) is active form, especially important for those with MTHFR gene mutations (~40% of population). Folic acid requires conversion.

What the evidence says — with links to the original studies
There is decades-long, high-quality randomized and systematic-review evidence that periconceptional folic acid (typically 400 mcg/day, higher for high-risk pregnancies) substantially reduces the risk of neural tube defects (NTDs), with the Cochrane review reporting a large risk reduction. Folic acid is the only folate form with proven clinical efficacy for NTD prevention; L-methylfolate (5-MTHF) has strong theoretical/bioavailability advantages and bypasses MTHFR variants but lacks clinical NTD-prevention trials. Debate persists over unmetabolized folic acid from high doses and the historical concern that folic acid can mask (correct the anemia of, but not the neurological damage from) vitamin B12 deficiency, though modern evidence finds routine 400 mcg dosing safe.
RDA 400 mcg DFE (600 mcg in pregnancy); folic acid ~85% bioavailable vs ~50% food folate; US NTD rates fell ~28% after 1998 fortification; discusses unmetabolized folic acid and B12 masking; UL 1,000 mcg/day.
5 trials, 7,391 pregnancies. Folic acid reduced NTDs, RR 0.31 (95% CI 0.17-0.58); recurrence RR 0.34. Protective across doses 0.36-4 mg.
Landmark multicenter RCT in high-risk women; folic acid produced a 72% protective effect against NTD recurrence, RR 0.28 (95% CI 0.12-0.71).
Folic acid shows 69%/66% NTD occurrence/recurrence risk reductions at 400 mcg; concludes there are NO published clinical studies of 5-MTHF for NTD prevention, so folic acid remains the only clinically proven form.
States 400 mcg/day folic acid is safe; 'more is not better'; unmetabolized folic acid circulates post-fortification but no confirmed health risks; fortification did not increase undetected B12 deficiency.
Argues folic acid should be replaced by 5-MTHF: folic acid is inert until DHFR-reduced, 5-MTHF bypasses MTHFR polymorphisms and avoids unmetabolized folic acid. A mechanistic argument, not a clinical trial.
Reviews the caution that high folic acid can mask/possibly worsen B12-deficiency neuropathy; low B12 + high folate correlates with worse cognition and higher homocysteine/MMA. Evidence is largely correlative and the hypothesis remains untested.
